Service boundary:

Preparation can begin before age 18 through coordinated transition planning. Taten Care’s Supported Living service is an adult pathway; under-18 CQC provision is Domiciliary Care only.

TRANSITION PATHWAY

A move that is planned, not rushed. The pathway preserves relationships, routines, communication and risk information so adulthood does not trigger an unnecessary cliff-edge in support.
